Guardian Emergency Contact Verification Form

Each year, ‘Guardian Emergency Contact Verification Forms’  go home to parent/guardians to ensure we have the most updated information regarding our students. This year's form reflects the data we currently have in our system, such as phone numbers, addresses, contacts, medical information, etc.

These yellow-coloured forms went home with students on September 25th to be reviewed and signed by a parent/guardian. 

Students should return the signed form to their Period 1 Teacher ASAP, and no later than Tuesday, October 14th. It’s important we receive these forms by this date so that we can make any necessary updates to your student’s personal information.

Important Notes:

  • There is a box to check off at the top of the form if there are no changes.
  • Make any changes directly on the form. 
  • If you have a change of address, change the address on the form and request a "Change of Address Form" from Office Assistant, Rylee Bourgaize, at rylee.bourgaize@ocdsb.ca.  
  • The student's phone is connected to 'Primary Contact # 1' and is not the student's personal phone number.
  • The form does not indicate if the student identifies as 'Indigenous'. Please add this information under the student's 'Preferred Name' if they self-identify as such.
  • Sign the form after you have verified all the information. 

We require this signed form back, even if there are no changes.
