If you are an alumnus/alumna of Canterbury High School who graduated/left this current school year or the previous school year and need a copy of your Ontario Student Academic Records (ie: transcript or diploma), please complete both of the following:
1. Fill out: Request for Ontario Student Academic Records
2. Scan a copy of Government-issued photo identification (ie: Health Card or Driver’s License)

If you are an alumnus/alumna of Canterbury High School who graduated / left more than a year ago and need a copy of your Ontario Academic Records (ie: transcript or diploma), please complete the three following steps:
1. Fill out: Request for Ontario Student Academic Records
2. Choose the appropriate link below to make payment through SchoolCash Online:
3. Scan a copy of Government-issued photo identification (ie: Health Card or Driver’s License)

If you cannot make payment online, the requested document, photo identification, and cheque can be mailed to:
